One heartbeat is 3 seconds long, also referred as a (combat) round.
You tick on each of your 10th heartbeat, usually coming in 30 sec intervals. Having faster than normal metabolism reduces this time.
Tick refreshes you per the regeneration abilities you have.
After combat, the first tick you get is called a combat tick and it will grant half what you would normally get.
